   b.	Cheque/Bank Draft/Postal Order (to be made payable to SMART Digital Communications Berhad) and send it to: Customer Care Centre SMART Digital Communications Berhad No. 31, Jalan Pelukis U1/46, Seksyen U1, Temasya Industrial Park 40150 Shah Alam Selangor Darul Ehsan, MALAYSIA
